The Department of Health is seeking your help in prioritizing public health educational messages over the upcoming year. The Department has created a short 10-question survey to assist in getting citizen input to make these priorities.

You can help by:

  1. Providing your input by completing the survey. It’ll take less than 10 minutes.
  2. Sharing the survey with your networks within the City including family members, friends, and neighbors. Using your influence to encourage members of your networks to complete would be a big help.

The survey includes a zip code question to help with understanding community thoughts about health promotion and health education in more limited geographic areas rather than city-wide.
